Honestly, ask her if she needs a break. When you ask her do it in a low stress situation and make sure you're honest with the offer.
DD needs to be more important than any commitment.

My opinion, get her with a skills coach that will re-build her confidence on the ball. Make sure he is also putting her in small sided games 3v3 2v2 to work on the skill developement in a game situation. Also dont panic it is fairly common for players to forget how they got to the level they are at! Low pressure skills training will build her back up. Be careful about putting her in leagues playing small sided, alot of her being scared comes from the increased pressure on players to play perfect every game and win. My two cents! if you need a good trainer low pressure great guy PM me!

Most importantly...Look at the path that got her to this point. If you see what is causing it that might help.
What position is she playing on the field? Is she feeling the pressure of clearing the ball or getting the ball passed up the line for a defensive position? Trying to speed up the attack as a mid-fielder by getting the ball to her forwards immediately rather than using speed/space? etc..
Do coaches or her teammates yell at her to pass almost as soon as she gets the ball?
Did her playtime get cut causing her to extra nervous to do well?
What does coach do in practice? Lots of passing and very little dribbling with the ball can cause some unseen effects. I generally can go to a Saturday game and she what the girls did at the practices that week. If coach worked solely on small sized passing and no shooting. The girls will pass the ball to death before someone actually shoots. If my dd did nothing but crossing in all week for the other girls to crush it then she wants to cross the ball the entire game instead of mixing it up.
Unfortunately my DD goes through this every fall. Coach sees the great crosses and wants those rather than her to actually do anything with the ball but dribble at full speed then cross the ball in to the forwards. Unfortunately her brain goes on auto pilot and she sometimes manages to out run everyone and cross into no one rather than take a shot. So every winter break we spend hours in camps/skills/indoor for her to remember that she can tackle, dribble, and shoot like everyone else.

I think it has made my dd more confident with her footwork and playing with her feet...she is a keeper and is playing the field for rec so it is a chance for something different for her. I have to tell her not to go over board at practice and games...she tries to use every skill she has ever learned just to show off...in a way it is good practice for her to use them in real situations/games but on the other hand, no one likes a show off.

Obviously, the root issue is that she is afraid to make a mistake. So I would investigate:
1. How does coach handle when players make a mistake? Does she get yelled at? Does he try to correct and encourage to do better next time? Does she get pulled and benched for the rest of the game (or even the next one!)?
2. How do her parents handle her mistakes (need a mirror here)
3. How do her teammates handle her mistakes? Do they point fingers and blame? Is there lots of team drama among the girls? Are there cliques that gang up on her and others?
I agree that skills work will build confidence. However, I have seen too many coaches, parents and teammates yelling at the poor DDs who are apparently the only ones who ever screw up as 12 year olds.
If the problem is #1 and #3, then it's time for a change. With #2, then you may or may not know what to do.

I agree with point #3. My DD was going through the same thing with her select team. Coach put her in a position that she is not used to playing. When she made a mistake, she got yelled at by her team mates. The thing that cured it for her, was her high school team. She loves playing on her high school team, and it shows. She has really picked up her game. I know that your DD is only twelve, but a change in teams might be the cure you are looking for. Bottom line, if she is not having fun, or enjoying playing, then a change needs to be made.
