Practice release form

Can someone tell me where to get the practice release form?

Noob gave you the full competitive release that would allow you to join another competitive team now. Good luck getting that signed.

If I understood the question, you are just looking to practice with another team while still playing with your current team. In most years, not many coaches would give their approval but with the move to calendar year a lot of coaches will not have a problem with this request. All you will need is for your current coach to write a letter that says they approve your child to practice with a specific team. There is not an approved form. The new team will want to keep the original letter with your current coaches signature.

Full release= You need the NTX form
Practice release= An email or note from your coach will do. There are no forms for practice release.

Just need an email from the coach to give to the team your practicing with. FYI not all clubs are being so generous even with the age change.

Correct. Here's the rule, pertinent section highlighted.

3.10 YOUTH CONTRACTS AND RELEASES 3.10.1
No coach, assistant coach, trainer, or team representative may practice any soccer related activity with any NTSSA registered player who does not appear on his current NTSSA roster or is not considered to be a free agent by current NTSSA Rules and Regulations except for a coach, who at the request of another teams coach, substitutes for that coach at practice or games for the convenience of the requesting coach. A free agent is a player in the U-11 through U-19 age division who desires to participate on a competitive team and is not rostered to any team. A competitive player may practice with another team if he has received a written permission letter signed by his current coach that sets out the dates and the team with which the player will practice. The player is to give the letter to the coach of the team with which he is practicing, and he must keep a copy for himself.
NOTE: The above does not include indoor soccer.

Methinks there's going to be a whole lotta letter writing. Parents need to get ready because these clubs are about to show their true colors this spring and summer.

North Texas updated their 'Open Practice' dates this year. So you have a couple of extra weeks to practice with other teams before tryouts barring any rain delays.
"After completion of State Cup competition for their age group or completion of league play for their team, whichever is last (the end of the soccer-playing year for competitive players), players may practice with other teams without any required release or other permission, although formal tryouts may not be held until July 1."

State Cup - Tournament Dates
April 28 - May 1, 2016 U11 thru U18
May 7 - 8, 2016 U11 thru U18 – Semi & Finals
June 1 - 5, 2016 U19
