Here's the problem. There is what you saw, and then there is what happened. It is hard to say what to call from a descriptive internet posting. But like was said before, it must be a purposeful pass back. Not a deflection, not a header, not a knee, but an intentional pass with the foot back to the keeper.
The ref has to judge the intent of the defender. If it is questionable at all, he isn't going to call it and give the opposing team a PK. When in doubt, don't make a call that could change the outcome of a game.
