Don't know about the free kick, but in the remaining 49 1/2 states, the player would receive a yellow card.

2. From the description given, it sounds like the ball wasn't thrown from over the head or to be more clear, it sounds like the player throwing, lost control of the ball before it was overhead. Therefore I believe that is not a legal throw in.
3. I think the correct answer is not listed. Throw in for the other team.

Throw-in taken by a player other than the goalkeeper
If, after the ball is in play, the thrower touches the ball again (except with his
hands) before it has touched another player:
• an indirect free kick is awarded to the opposing team, to be taken from the
place where the infringement occurred (see Law 13 – Position of free kick)
If, after the ball is in play, the thrower deliberately handles the ball before it has
touched another player:
• a direct free kick is awarded to the opposing team, to be taken from the
place where the infringement occurred (see Law 13 – Position of free kick)
• a penalty kick is awarded if the infringement occurred inside the thrower’s
penalty area
So I would say direct kick in this case...

The thrower shall use both hands (unless because of a physical impairment can only use one hand) and deliver the from behind and over the head in one continuous motion.
Key points here is that the ball must be thrown with both hands (unless a physical impairment exists).
Both hands must be on the ball and come forward with the ball to make the throw.
Watch for throws that are illegally made with one hand rather than both hands.
The throw has to start behind the head, come over the head and then release the ball.
I'll stick with my, not a legal throw.
Throw in for the other team.

Soccernovice although no referee got it correct. The direct kick ended up as a shot on goal and resulted in a goal.
The opposing coach wanted an indirect kick since it was a double touch by the player. However, it was the touch with the hands that changes the call of the play.
